Window Buttons Broken on New Dr460nized Install (used 0428 NVIDIA ISO)

I have a new Dr460nized install (0428 NVIDIA ISO), and after updating everything, I rebooted. Now, whenever I log in (does not matter if X11 or Wayland), the window buttons applet is broken. I just get the following icon:

that shows at all times, even when a window is not maximized.
When left clicking on the applet, I get the following error:

When right clicking I get the following menu, does not give me options to edit window button specific options:

I have not made a ton of config changes or anything. All I did was simply install it, run through setup-assistant (I did install the LTS and Clear kernels in addition to zen), and reboot.

Is there a way I can fix this?

I’ve had really hit or miss experience on copying text that is located in the KDE system itself. Like, sometimes when I hit Ctrl+C it’ll copy, sometimes it won’t. So I got in the habit of taking screenshots.
Here is the copied text:
